Behavioral health billing is mostly a matter of minutes and licenses. A 52-minute therapy session and a 53-minute session bill under different codes, and the same session pays differently depending on whether a psychologist, an LCSW, or a counselor provided it. Add payers that carve behavioral health out to a separate plan, and a practice can be credentialed with a patient’s insurer and still be out of network for that patient’s therapy.
Key Takeaways
- Psychotherapy codes bill by face-to-face time, with 90832 for 16 to 37 minutes, 90834 for 38 to 52 minutes, and 90837 for 53 minutes or more.
- Licensed professional counselors and marriage and family therapists have been able to enroll in Medicare since January 2024, at 75% of the clinical psychologist fee schedule.
- Medicare’s in-person visit requirement for mental health telehealth is delayed through 2027, and audio-only therapy to the patient’s home remains billable.
- Psychological testing now bills under 96130 to 96139, and the older 96101 and 96102 codes no longer exist.
- Many health plans route behavioral health to a separate managed behavioral health organization that credentials providers on its own.
Psychotherapy and Evaluation Codes
Most outpatient behavioral health claims start with an intake and continue with psychotherapy. The diagnosis on every claim is an ICD-10-CM code, even when the clinician’s assessment is written in DSM-5-TR terms, and a DSM label without its matching ICD-10 code is a common reason for rejections.
90791 for a psychiatric diagnostic evaluation without medical services, and 90792 when medical services are included- 90832, 90834, and 90837 for individual psychotherapy at 30, 45, and 60 minutes
- 90833, 90836, and 90838 as psychotherapy add-ons when a prescriber also bills an E/M visit
- 90846 and 90847 for family psychotherapy without and with the patient present
- 90853 for group psychotherapy
- 90839 and 90840 for psychotherapy for crisis, starting at 30 minutes
- 90785 for interactive complexity, added when a third party or communication barrier changes the session
Time rules decide most psychotherapy denials. The clock counts only face-to-face time with the patient or family, so a session documented as “60 minutes” that started late doesn’t support 90837 unless the note shows at least 53 minutes. Some commercial payers also audit heavy 90837 use or require authorization after a set number of sessions. Our guide to behavioral health CPT codes lists the full set, and our post on behavioral health modifiers covers the license-level and telehealth modifiers payers expect.
Testing, Health Behavior, and Newer Services
Psychological and neuropsychological testing has billed under a newer code structure since 2019. Evaluation services by the psychologist bill as 96130 and 96131 for psychological testing or 96132 and 96133 for neuropsychological testing, and test administration and scoring bill under 96136 through 96139 depending on who gives the test. Health behavior assessment and intervention for patients whose medical conditions have a behavioral component bills under 96156 through 96171.
Medicare has added several codes in the last two years that behavioral health groups often miss. Safety planning interventions for patients at risk of suicide or overdose bill as G0560, and follow-up calls after an emergency department visit for a behavioral health crisis bill as G0544. Digital mental health treatment devices bill under G0552 through G0554, and for 2026 Medicare extended those codes to FDA-cleared ADHD treatment devices used alongside ongoing care. Our digital therapeutics page covers how device-based treatment is billed.
Practices that work inside primary care can bill through the medical practice instead. General behavioral health integration (99484) and the psychiatric collaborative care codes (99492 to 99494) are billed by the treating primary care practitioner, with the behavioral health care manager’s time counting toward the monthly code.
Medicare, Medicaid, and Carve-Outs
Medicare pays psychiatrists and psychiatric nurse practitioners under the regular physician fee schedule, while clinical psychologists, clinical social workers, licensed professional counselors, and marriage and family therapists each have their own enrollment type. Counselors and MFTs were added in January 2024 and are paid at 75% of the clinical psychologist rate. Behavioral health services from auxiliary staff can be billed incident to a supervising practitioner under general supervision, so the supervisor doesn’t have to be on site.
Medicaid is where most behavioral health volume sits, and it’s also where the rules vary most. Many states carve behavioral health out of their main managed care contracts and send it to a separate organization, and commercial plans often do the same through a managed behavioral health organization. A clinician can be in network with a patient’s medical plan and still be out of network for therapy because the plan’s behavioral benefit runs through another company. Practices that also treat substance use disorders should see our substance abuse billing page, and psychiatry groups have their own page on psychiatry billing.
Telehealth for Behavioral Health
Behavioral health is the one area where Medicare made telehealth to the patient’s home permanent. What changed in 2026 is the timing of the in-person rule. The Consolidated Appropriations Act, 2026 pushed Medicare’s requirement for an in-person visit within six months before starting mental health telehealth, and every 12 months after that, out through the end of 2027. Audio-only sessions remain billable when the patient can’t or won’t use video, as long as the clinician is capable of video and the note says why audio was used.
Commercial and Medicaid telehealth rules are set state by state and plan by plan. Most require the clinician to be licensed in the state where the patient sits during the session, which is why multi-state groups use the psychology, counseling, and social work compacts where their states belong. Our post on the 2027 telehealth extension covers what happens if Congress doesn’t act again.
Behavioral Health Credentialing

License types and what each can bill
Payers credential each license type differently. Psychologists, LCSWs, LPCs, and LMFTs are usually credentialed as independent practitioners once fully licensed, while associates and pre-licensed clinicians generally can’t bill under their own NPI. Some commercial and Medicaid plans allow their services to bill under a licensed supervisor with a specific modifier, but Medicare doesn’t enroll them at all, so supervised hours have to be billed carefully or not billed to Medicare.
CAQH, carve-outs, and panel closures
Most commercial plans and behavioral health organizations pull applications from CAQH ProView, now part of DataSpring, so the profile has to be complete and reattested every 120 days. Our post on keeping a CAQH profile current covers why a lapse stops claims. Behavioral health panels close more often than medical panels, so a clinician may be told a network is full in one county and open in the next, and an application can be accepted only after the practice shows an access gap. The full process is laid out in our guide on getting credentialed as a mental health provider.
Group practices and new clinicians
Group practices need every clinician linked to the group’s tax ID with every payer, and each new hire repeats the process. Claims for a clinician who isn’t yet active with a payer either deny or have to be held, which is why credentialing for a new therapist should start the day an offer is signed.
Behavioral Health Billing FAQ
Which psychotherapy code fits a 53-minute session?
CPT 90837, which covers 53 minutes or more of face-to-face psychotherapy. A session of 38 to 52 minutes bills as 90834, and 16 to 37 minutes bills as 90832.
Can licensed professional counselors and MFTs bill Medicare?
Yes. Since January 1, 2024, LPCs, licensed mental health counselors, and marriage and family therapists can enroll in Medicare and bill under their own NPI. They’re paid at 75% of the clinical psychologist fee schedule.
Do Medicare telehealth therapy patients need an in-person visit?
Not through the end of 2027. The Consolidated Appropriations Act, 2026 delayed the in-person requirement for mental health telehealth, and it’s scheduled to return in 2028 unless Congress extends it again.
Can a prescriber bill psychotherapy and an E/M visit on the same day?
Yes. Psychiatrists and psychiatric nurse practitioners bill the E/M visit and add 90833, 90836, or 90838 for the psychotherapy time, which has to be documented separately from the time spent on the medical portion.
Can pre-licensed therapists bill insurance?
Usually not under their own NPI. Some commercial and Medicaid plans allow their services to bill under a licensed supervisor, but Medicare doesn’t enroll pre-licensed clinicians, so the rules depend on the state and the payer.
What replaced psychological testing codes 96101 and 96102?
Since 2019, psychological testing bills under 96130 and 96131 for evaluation services and 96136 through 96139 for test administration and scoring. Neuropsychological testing evaluation bills as 96132 and 96133.
Medwave for Therapists, Counselors, and Psychologists
Behavioral health claims rarely fail on the code itself. They fail on session time that doesn’t match the note, a license type the payer doesn’t recognize, or a carve-out plan nobody checked. Medwave has worked with therapy groups, psychologists, and counseling practices since 2000, and our medical billing team holds a 98% clean claim rate, with denial management working anything that comes back.
Our medical credentialing team enrolls each clinician with the medical plan and the behavioral health carve-out at the same time and averages 85 days to approval. For practices still weighing whether to bring billing in house, we wrote up the advantages of outsourcing behavioral health billing.
